Our vision is to provide patients with a Patient-Centered Medical Home (PCMH) where comprehensive evidence based care is provided and trusting partnerships are fostered between our physician-lead healthcare team and our patients.

The PCMH model is an approach to providng care which strengthens the physician-patient relationship by replacing episodic care with coordinated care and a long-term healthcare relationship.  The PCMH model seeks to foster a relationship of trust between the healthcare team and the patient, and to actively engage patients as partners in their healthcare.

To effectively serve as your medical home, it is important that we have the most current up-to-date information about your healthcare such as medications including over the counter; visits to spcialists, medical history, health status, recent test results, self-care information, and data from recent hospitalizations, speciality care, urgent care or ER visits.

Scotia Glenville Family Medicine (SGFM) is currently certified by the NCQA (National Committee for Quality Assurance) as a Level 3 PCMH.  In addition, SGFM is participating in a  Comprehensive Primary Care initiative with the Centers for Medicare & Medicaid Services.  This program, like the PCMH program strives for better care, better health outcomes, and reduced cost of care through improvement in care. SGFM chooses to participate in these programs as we are continually seeking to improve the quality, effectiveness, efficiency of the care we deliver and to increase staff-patient satisfaction using these models.    

Scotia Glenville Family Medicine takes pride in valuing the opinions of our patients.  We have a Quality Improvement Committee that reviews our patient satisfaction survey results and works to improve processes to increase patient satisfaction.  Some highlights of areas we have worked on include: 
  • Contacting patients who are overdue for preventative and chronic care services
  • Improved access to care by implementing a walk-in clinic Monday through Friday mornings
Our most recent patient satisfaction scores reflected improvement in each of these areas. 

In an effort to further improve the quality of care that we provide to our patients, we are happy to announce the formation of our Patient Advisory Committee.  The committee will comprise of 8-10 patients and 3-4 staff members who will meet on a quarterly basis; we look forward to working with patients to help identify and improve areas of concern.

7:30 a.m. - 8:00 a.m.
Saturday: 8:30 a.m. to 12:00 p.m.
